This is your nomination form for the 2018-19 All-State, All-State Academic (individual & Team awards), MHSHCA Scholarship, Coach of the year, Assistant Coach of the Year & Friend of High School Hockey Awards. As you answer yes to questions you will be prompted to fill out that info and if you hit no you will be prompted to the next question. This is for MHSHCA members that are Head Coach's only. You may nominate up to 3 Forwards, 2 Defenseman & 1 Goalie for All-State (you do not have to nominate players for all of available positions).  The academic awards will still need your AD or Administrator signature with your signature on the downloadable forms, you WILL NOT need to send transcripts for your players anymore. You will email these completed forms to aqmhshca@gmail.com. You will need both the weighted and unweighted GPA's (we will not use the weighted GPA's for the unweighted awards) if you do not submit the correct GPA's your player will not be put in for the award.  The MHSHCA Scholarship is one Sr nomination from each team, it is a $500 scholarship that will be randomly drawn at the year end meeting per division.  You must download and print the below attachments for those nominations before you continue.  Do not hit autofill if your computer prompts you to use that feature.
